The tear trough is the groove that runs from the inner corner of the eye towards the cheek. As we age — or in some patients from an early age — this groove deepens, creating a shadow that makes us look persistently tired or older than we are. Tear trough filler is one of the most technically demanding injectable treatments available. The under-eye area has complex vascular anatomy and the skin is extremely thin — making the choice of clinician critically important. Detailed Assessment

Cannula Technique

A blunt-tipped cannula (rather than a sharp needle) is used for tear trough filler — reducing the risk of bruising and allowing more controlled placement of filler in this sensitive area.

Before

Avoid blood thinners for 48 hours. Come to your consultation with no makeup.

During

Topical anaesthetic applied. Cannula technique used. 20–30 minutes.

After

Mild bruising or swelling for 3–7 days in the eye area. Sleep elevated. Avoid pressure on the area.

Results

Immediate improvement in hollowing. Final result at 1–2 weeks once swelling settles completely.

Longevity

12–18 months. Individual variation applies — the eye area can metabolise filler faster in some patients.

Am I a suitable candidate for tear trough filler?

The best candidates are patients with a clear structural hollow contributing to shadows. Patients with very thin skin, significant puffiness (eye bags), or predominantly pigmented dark circles may not be ideal candidates. A detailed consultation is essential.

Does it hurt?

Topical anaesthetic ensures comfort. The cannula technique is generally less uncomfortable than needle injection in this area.

Why is the tear trough technically demanding?

The under-eye area has thin skin, complex vascular anatomy and very little tolerance for overfilling. Too much filler, the wrong product, or incorrect placement can lead to visible lumps or the Tyndall effect (bluish discoloration). Selecting an experienced, medically qualified clinician is essential.

Can polynucleotides be used instead of filler under the eyes?

Yes — polynucleotides are an excellent alternative or complement for patients whose under-eye concerns are related to skin quality rather than structural hollowing. Many patients benefit from a combination approach.
